Cramps, Tense Muscles, Sciatica, Pain, or Tingling

Pain and a variety of abnormal sensations may be due to PD. These especially are experienced in the limb or side of the body most affected by PD. These sensations tend to be at least partially responsive to carbidopa/levodopa.

CRAMPS, TENSE MUSCLES

Almost everyone has experienced cramps, such as after vigorous exercise. Cramps are especially common in PD, and in that context are caused by the brain dopamine deficiency state. They respond to adequate carbidopa/levodopa treatment. PD cramps may affect the calves, but especially the toes, manifesting as toe-curling or upward toe deviation. PD cramps presumably represent a form of dystonia and may develop during levodopa off-states. Some people recognize cramps as a signal to take their next dose of levodopa.

Nighttime cramps often occur because of the relatively long interval since the last dose of carbidopa/levodopa. These nocturnal cramps respond to carbidopa/levodopa coverage, which is discussed in more detail in Chapter 20, on sleep.

Tense muscles are also common in PD and occur especially in the limb(s) first affected by PD. The tension typically improves with carbidopa/levodopa therapy, but the response may be incomplete.
